The Environmental Health Investigations Branch is looking for a Contractor to deliver the Asthma Management Academy training to community health workers (CHWs) that provide (or will provide) asthma self-management and environmental trigger education. The Academy training shall be delivered by CHW facilitators, performed in English and Spanish, virtually or in-person, with organizations (e.g., federally qualified health centers, Medi-Cal managed care plans, and community-based organizations) serving high asthma burden communities ¿ communities identified by California Breathing as being located in census tracts in the 75th percentile for asthma-related Emergency Department visits and hospitalizations. These are predominantly Black, Latino, and Native communities. Counties with the highest asthma disparities are Alameda, Contra Costa, Del Norte, Fresno, Imperial, Kern, Kings, Los Angeles, Madera, Merced, Monterey, Riverside, Sacramento, San Bernardino, San Diego, San Francisco, San Joaquin, San Mateo, Santa Clara, Solano, Stanislaus, and Tulare counties. The Contractor shall conduct six 20-hour Academy training sessions (either virtually or in-person, and in English and Spanish), led by a Community Health Worker (CHW) facilitator to priority organizations, including federally qualified health centers, managed care plans, community clinics, non-profits, school-based health centers and community-based organizations conducting social services, asthma home visiting, or education services. Proposals must address all the services described in Exhibit A, Scope of Work. CDPH intends to make a single contract award to the most responsive and responsible firm offering the highest scoring bid. This procurement is open to all eligible firms and/or individuals that meet the qualification requirements, including commercial businesses, nonprofit organizations, State or public universities (including auxiliary organizations) and other entities.
