1. RFQ form 356803-JB - highlighted sections completed

2. PUR-466 (Rev 1.)* – Proposal Certification – all applicable sections completed and form signed.

3. SARC Subcontractor Annual Reps & Certs (Rev. 3)* – completed and signed.

4. A separate document including any/all exceptions to FRA LLC General Terms and Conditions for Commercial Products and Commercial Services (03.2024)*. Exceptions to FRA Terms and Conditions shall be presented with RFQ submittal.

* Not required/applicable if GSA terms are submitted
