The City and County of San Francisco, San Francisco Public Works (Public Works) seeks a Statement for Qualification for qualified vendors to perform supportive services for the City in the event of an emergency at various locations in San Francisco, California. An emergency is defined in Section 6.60 of the San Francisco Administrative Code. Vendors who are interested in being placed on this Emergency Supportive Service Prequalified Vendorsâ List should submit a Statement of Qualifications, and consisting of cover letter requesting to be placed on the list and the completed Questionnaire. (attached below). Submit the cover letter & questionnaire by regular mail or email to: San Francisco Public Works Contract Administration 1155 Market Street, 4th Floor San Francisco, CA 94103 San Francisco Public Works will accept Statement of Qualifications on a continuous basis, but reserves the right to terminate the solicitation any time. The City reserves the right to reject any or all Statement of Qualification(s) and waive any minor irregularities. Once placed on the list, the Vendor must notify the City of any changes to the information provided. Vendors shall update their information by submitting a new Questionnaire with the updated information clearly highlighted to the address listed below. No emergency supportive work may be performed over the contracted not to exceed amount unless authorized by Public Works. The City reserves the right to terminate the emergency agreement at any time. Vendors must agree to perform emergency supportive work and will be required to respond and mobilize to perform work at a job site within 2 to 4 hours. Vendors must also submit valid insurance certificates prior to start of emergency supportive work. The attached Questionnaire was revised on 10/6/2017 to add new information.
