Business size, type, and SAM Unique Identifier Number Affirmation that the vendor is capable of performing in accordance with the attached Requirements Description. Provide Surgical Pathology Pre-Op vs. Post-Op Report quarterly. Electronic Interface. Meet Turnaround Times per Statement of Work. Provide Phone Contact List. Maintain a 95% or higher Proficiency Testing rate. Provide copies of all licenses, permits, accreditation, and certificates required by law to do the work described in the Requirements Description. Provide fully functioning interface to Vista A, Cerner, Future Systems.
