Q2) Who will the Contractor set up specific times to pick up the daily patient pathology specimens with? Lynn Burke? R2) Pickup address and location have been updated in version 2: S02 RFQ_ Attachment B_v2. Q3) Can the Contractor return all pathology materials after the results have been transmitted to SAVAHCS via courier services? If they can be returned via US Mail, are they to use: Dept of VA, Southern Arizona Healthcare System, Warehouse, 3601 S. 6th Ave, Tucson, AZ 85723, or VASAHS laboratory & Pathology Dept, Attn: Lynn Burke, 3601 S.  6th Ave, Tucson, AZ 85723? R3) Return address, location, and trackable delivery source requirements have been updated in version 2: S02 RFQ_ Attachment B_v2. Q4) If additional testing is identified by the Dermatopathologist, where can those charges be identified by the Contractor on the price/schedule list? R4) If additional testing (not covered under this contract) is identified by the Dermatopathologist, SAVAHCS will determine if extra testing is necessary and authorized payment via Government Purchase Card. This information has been updated in version 2: S02 RFQ_ Attachment B_v2. Q5) Can you please provide us with more details regarding the specific tests needed in the pricing schedule. R5) The CPT codes indicate what the tests are. These tests are standard laboratory tests performed in a fully licensed/accredited dermatopathology lab, actively engaged in providing these tests and services, under the supervision of a dermatopathologist. Revision 2 of Attachment B Statement of Work (SOW) is attached.
