This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. A separate solicitation will not be issued. The solicitation is issued as a Request for Quotation (RFQ) and is solicited and will be awarded IAW FAR Part 13. This combined synopsis/solicitation incorporates provisions and clauses in effect through Federal Acquisition Circular 2021-07 and DFARS change notice dated 10/29/2021. This is a requirement for a 3Shape E4 Dental Scanner for Joint Base San Antonio (JBSA) 59th Medical Wing, located JBSA-Lackland, Texas.

The vendor shall respond to this RFQ by: 19 November 2021; 10:00am CST via email to Contract Administrator: Jason Wilburn, email [email protected]. Late submission will be treated IAW FAR 52.212-1(f).

This is a Firm Fixed Price acquisition is 100% Set-Aside to Small Business concerns, utilizing NAICS 339114, Business Size Standard, 750 Employees.

(Upgraded software is purchased and will have to have the dongle transferred if by a different company)

1. This is a notice that this purchase order is solicited as a Small Business Set-Aside under NAICS 339114, therefore only small businesses may submit a quote.
2. Questions/Concerns. Shall be submitted via email to [email protected] not later than 19 November 2021, Friday by 10:00 a.m. CST.
3. Basis for Award.  This will be a best value/trade off award decision. The basis for award decision will be made on providing the product requested, shipping lead time, and price. Additionally value may be added to accessories for the scanner.
4. Mandatory Registrations. To be eligible for award, registration with the System for Award Management (SAM) must be current (legacy for On-line Registration and Certifications Application (ORCA) and Central Contractor Registration (CCR) registrations). To register or update former CCR and Reps & Certs that are not current go to https://beta.sam.gov or at www.sam.gov and provide mandatory information.
5. Location Area:  59th Medical Wing, 1100 Wilford Hall Loop, Bldg 4554 Charles F De Freest, San Antonio, TX 78236.
