The Southern Arizona VA Health Care System (SAVAHCS), 3601 S. 6th Avenue Tucson, Arizona 85723, has a requirement for a Novus Varia Diode Pumped Solid State Photo-coagulator Laser System (0642-00-01) and its accessories for the treatment of Veteran Patients with ocular hemorrhage, ocular melanoma, retinopathy, and other ocular abnormalities. The Photo-coagulator Laser System shall deliver energy settings of 50mW to 1500mW Green, 50mW to 600mW Yellow, and 50mW to 600mW Red Wavelengths to meet the treatment requirements. The Three-color Laser System shall also include a footswitch, a remote control, safety glasses, user-operator_s manual, and a laser safety sign. The SAVAHCS also requires the Novus Varia Zeiss SL130 Kit Filter (0642-715-01), the Lio Varia Kit (0642-170-01), and the 4-Filter Auto LL (0642-732-01) Kit for use with the Laser System. The Contractor shall allow an equipment trade-in credit for receipt of SAVAHCS used equipment. The Contractor must provide a 24-month all-inclusive warranty for the equipment. The North American Industry Classification (NAICS) code for this requirement is 339112. This requirement is a 100% Small Business Set-aside. The SBA size standard for this NAICS code is 500 Employees. Proposals submitted from other than Small Business sources will NOT be considered. The SAVAHCS anticipates awarding a firm-fixed price contract for this effort. This requirement will be procured utilizing procedures in the Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items and Part 15, Contracting by Negotiation.
