This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quotation (RFQ), solicitation number, VA-258-10-RQ-0384, in accordance with FAR parts 12 and 13, and is in accordance with provisions and clauses effective through Federal Acquisition Circular 2005-43. Additionally, this is a brand name specification for Codman & Shurtleff, Inc manufactured product only, IAW FAR 6.302-1( c) Only one responsible source and no other supplies or services can meet the agencies needs. North American Industry Classification System (NAICS) number is 423450 and small business size standard is 100 employees. The proposed acquisition is set aside for Small Businesses.
Description of requirements for the items to be acquired:
Item 1: Codman Malis CMC III (manufacture's product number 80-1171) quantity two each
Operators, service and repair parts manuals, quantity four each.
Items are required no later than 30 days after receipt of order, FOB Destination: Department of Veteran Affairs, Southern Arizona VA Health Care System, Building 14, POC: Rick Harker ext 5508, 3601 S. 6th Ave., Tucson, AZ 85723.
