This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is W81K00-14-T-0142. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72. The North American Industry Classification System Code (NAICS) for this procurement is: 339112; the Standard Industrial Classification Code is 3842; and the small business size standard is 500. This requirement is (UNRESTRICTED) and has a requirement for the following:

Establish a Blanket Purchase Agreement to provide all arthroscopy medical supplies for the Surgical Department. These medical supplies will be used by the surgeon for hip implants for the Arthroscopic procedure. Pivot Medical is a leader in the Hip Arthroscopy and using distinct instrumentations. All items will be called for as needed and deliver to Eisenhower Army Medical Center, Fort Gordon, GA. The required supplies and instruments shall be manufactured by Pivot Medical. Payment will be made by 25K Purchase Card. The period of performance will be from the signature date of the Contracting Officer through 30 September 2019.
