The requirement is estimated for a Base period of March 01, 2012 thru February 29, 2012 plus 4 option years. Quantities are estimated at 180 cases to be ordered "as needed basis". The custom packs must include various lengths of ¼, 3/8, and ½ in tubing, tubing end caps, one way valves, clamps, and stopcocks; various tubing connectors; prebypass and arterial filters; cardiotmy reservoir; hempconcentrator. The total pack will also contain one oxygenator with hard-shell venous reservoir.
