Sources Sought Notice Description: This is not a solicitation announcement. This is a sources sought synopsis only. The purpose of this sources sought synopsis is to gain knowledge of potential qualified sources and their size classifications relative to NAICS code 339112 (size standard of 500). The Government shall use responses to this sources sought synopsis to make appropriate Acquisition decisions-see submittal requirements below. After review of the responses to this sources sought announcement, a solicitation announcement will be published in the FEDBIZOPPS. All interested parties will have to respond to the solicitation announcement (Request for Proposal) in addition to responding to this sources sought announcement. The Louis Stokes Cleveland Department of Veterans Affairs, VISN 10, is seeking sources for a potential contract to provide Cryocath Cryoablation system for the Louis Stokes Cleveland VA Medical Center. Description of item: Cryocath Cryoablation system is intended to destroy (ablate) abnormal heart tissue to treat drug refractory paroxysmal atrial fibrillation (PAF), an intermittent abnormal heartbeat in the upper chambers of the heart that cannot be treated with medicines. The Arctic Front® Cardiac CryoAblation Catheter is a steerable, flexible plastic tube (catheter) with a balloon at its tip that expands when filled with coolant delivered from the CryoConsole, a computerized control center. The Freezor® MAX Cardiac CryoAblation Catheter is a catheter with a metal tip, rather than a balloon, to which coolant can also be delivered. The Manual Retraction Kit provides the doctor with an alternative method of removing the Arctic Front balloon catheter from the patient's heart, if needed. Submittal requirements below: Responses to this notice should include company name, address, point of contact, cage code, size of business pursuant to North American industrial classification system (NAICS) and must respond to the following questions: (1) is your business a large or small business? (2) If small, does your firm qualify as a small, emerging small business, or a small, disadvantaged business? (3) If disadvantaged, specify under which disadvantaged group and is your firm certified under section 8(a) of the small business act? (4) Is your firm a certified "hub zone" firm? (5) Is your firm a woman-owned or operated business? (6) Is your firm a certified service-disabled veteran owned and veteran-owned? This is not a request for proposals and in no way obligate the government to award any contract. Responses to this notice shall be in writing by e-mail addressed to [email protected] or by fax (216) 447-8310. Responses must be received in writing no later than 12 noon E.S.T. on Friday, September 7, 2012. Responses must include VA250-12-I-0832, in the subject field. Telephone responses will not be accepted. Firms responding to this announcement, which fail to provide all of the required information, cannot be used to help the government make the acquisition decisions, which is the intent of this synopsis. This notice is to help the VA medical center in determining potential sources only. No solicitation is currently available. All firms responding to this sources sought notice are advised that their response to this notice is not a request to be considered for a contract award. If a solicitation is issued, it will be announced later in the FEDBIZOPPS and all interested parties must respond to that solicitation announcement separately from the responses to this announcement.
