DESCRIPTION
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, and FAR subpart 13, Simplified Acquisition Procedures, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued.
This solicitation is a request for quotations (RFQ). The government will issue a purchase order resulting from this request to the responsible vendor whose quote will be the most advantageous to the Government, price and other factors considered. Items are to be delivered FOB destination, 30 days After Receipt of Order (ARO). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-03, effective 12 June 2019.
The associated North American Industrial Classification System (NAICS) code for this procurement is 339112, Surgical and Medical Instrument Manufacturing with a size standard of 1,000 employees. This solicitation is a 100% set-aside for Service Disabled Veteran Owned Small Businesses (SDVOSBs) / Veteran Owned Small Business (VOSB). Quoters must be verified in the Vendor Information Pages (VIP), https://www.vip.vetbiz.va.gov/, at the time of receipt of quotes and at the time of award.
The Department of Veterans Affairs Program Contracting Activity Central at 4401-D Meramec Bottom Road, St. Louis, MO. 63129 is soliciting quotations from SDVOSBs for Repair Parts for Laerdal Adult and Infant Voice Activated Manikins. The contractor shall provide all personnel, equipment, tools, materials, supervision, and services necessary to provide Repair Parts for Laerdal Adult and Infant Voice Activated Manikins for the Department of Veteran Affairs.
All interested companies shall provide quotation(s) for the Services described in the attached Purchase description / Statement of Work

BASIS OF AWARD
Award will be made based on the item(s)/service(s) offered that best meets Governments requirement, considering technical acceptability and price. Quotes will be reviewed by qualified evaluators to initially determine their acceptability. Each quote must contain all the information required by the RFQ. A quote may be determined to be unacceptable if required information is missing or if the quote materially deviates from the requirements of the RFQ. Unacceptable quotes will not be considered for further evaluation or selection. A responsibility determination will be made in accordance with FAR 9.1, Responsible Prospective Contractors.
QUOTATION SUBMISSION INSTRUCTIONS
All Quotation Packages submitted must include:

Quoter POC information to include: name, title, telephone number and email.
The Quoter shall submit their Capability Statement.
Completed Price Cost Schedule for Firm Fixed Price to include: Quantity, Unit, Unit Price and Total Amount.
Failure to provide complete pricing, including pricing for each CLIN, will result in the contractor s package being determined to be incomplete.
Quoters that fail to furnish required representation or information, or reject the terms and conditions of the solicitation, may be excluded from consideration.
Teaming Arrangements If a Teaming Agreement or other Teaming Arrangements (as defined in the section entitled Teaming Agreements and Other Teaming Arrangements of this combined synopsis/solicitation) is contemplated, the arrangements MUST be identified, and company relationships must be fully disclosed in the quotation package submitted.
Please describe the division of responsibilities between the prime contractor and the subcontractor and definition of the relationship of the parties. The parties should clearly allocate the scope of work between the prime contractor and the subcontractor. At a minimum, the parties should also clarify:
Their respective responsibilities in the creation and submission of the quotation to the federal government;
If the relationship between the parties is that of independent contractors and therefore neither party has the right to bind the other party.
The Government contemplates award of a Firm Fixed Price (FFP) contract resulting from this solicitation.
