Purpose and Objectives: The intent of this Sources Sought Notice is to identify potential offerors capable of providing Gambro supplies: dialyzers, biocart 1250g, blood set, Polyflux Revaclear Dialyzer and Polyflux Revaclear Max. Facility does not have loading and unloading and has limit space for deliveries. Place of Performance: VA Fayetteville Dialysis Unit, North Carolina 28305. Response: Responses to this Sources Sought Notice should demonstrate the firm's ability, capability, and responsibility to perform the principal components of work listed above. Responses should include the following information: Business name, address, business size and Socioeconomic Category (HUBZone, 8(a), Service Disabled Veteran, Small Business, Large Business, etc.), and point of contact to include e-mail address and telephone number. All information is to be submitted via e-mail at [email protected]. Information provided will not be returned. All responses shall be in the English Language. Responses are due by 3:00 pm (EST) on Friday May 13, 2011. No submissions will be accepted after this date and time. Questions can be submitted electronically to [email protected].
