The VA Puget Sound Health Care System, Seattle WA intends to negotiate a sole source contract for the following Codman manufactured surgical instruments: 501516 BAR,SECONDARY FLOATING 501512 HAND REST 501513 GREENBERG PATTIE 501507 BAR & CLAMP 501508 BAR, SECONDARY 501509 ARMS,LONG 501514 STERILIZING CASE SP01031 OLD STYLE GREENBERG BLADE (CUSTOM BLADE) SP01031-1 OLD STYLE GREENBERG BLADE 50-1503 (CUSTOM) CE MARKED-LABELS FOX PRO SP01031-2 OLD STYLE GREENBERG BLADE 50-1502 (CUSTOM) CE MARKED-LABELS FOX PRO SP01031-3 OLD STYLE GREENBERG BLADE 50-1504 (CUSTOM) CE MARKED-LABELS FOX PRO SP01031-4 OLD STYLE GREENBERG BLADE 50-1505 (CUSTOM) CE MARKED-LABELS FOX PRO SP01031-5 OLD STYLE GREENBERG BLADE 50-1506 (CUSTOM) CE MARKED-LABELS FOX PRO This requirement is unique in that Johnson and Johnson is the only authorized distributor of the required Codman products. This proposed action in which the Government intends to solicit and negotiate with only one source is being conducted under the authority of FAR 6.302-1(b) (1) (i). This notice of intent is not a request for competitive proposals. No solicitation document is available and telephone requests will not be honored. If however, any source believes it can furnish the service described above it is encouraged to furnish information about its product and services, as well as references from other customers who are using the same product.
