Zablocki VA Medical Center, Milwaukee, WI. Mizuho Orthopedic Systems, Inc. is the only source for this product. This procurement is being conducted in accordance with FAR 6.302-1, Only one responsible source and no other supplies or services will satify agency requirements. Contract is firm fixed price and F.O.B. Destination.
