SYNOPSIS: The Department of Veterans Affairs, VISN 6 Network Contracting Office, on the behalf of the Salisbury VA Medical Center has a need for Olympus/ Gyrus Cyberwand Dual Ultrasonic Generator system and Cyperwand Wrench. This is a Brand Name or equal requirement. This procurement is for new equipment only; no remanufactured or "gray market" items. All items must be covered by the manufacturer's warranty. SOLICITATION: See attached solicitation VA246-14-Q-0139. This solicitation is a SDVOSB set-aside. Only VetBiz registered SDVOSBs will be considered for award. Quotations shall include the RFQ number, the proposed unit price, the proposed extended price, any prompt payment discount terms, any shipping charges, projected delivery date, company name, and point of contact, address, phone number, fax number, tax identification number, and DUNS number. Vendor must be registered in SAM (System for Award Management) and VetBiz. Lack of registration in the SAM database will make an offeror ineligible. To register, vendors may go to https://www.sam.gov via the internet. This procurement is a SDVOSB set-aside therefore vendor must also be registered at VetBiz at http://vetbiz.gov to be considered for award.
