I. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Extraction instrument sets must be designed to remove any manufacturer's hardware ranging in size from 1.5mm-femoral nails. 2. The instrument sets must be designed to remove any manufacturer's broken or stripped hardware in size from 1.5mm-femoral nails. 3. The vendor shall state the manufacturer/brands and sizes of hardware the offered extraction set successfully removes, to include broken or stripped screws. Documentation is requested to verify claims. 4. The instrument sets must be reusable. 5. The vendor will conduct inservices for OR staff, SPS staff, and Logistics staff during normal working hours and designated training times. Training will cover, at a minimum, using the tools in podiatry and orthopedic cases, sterilization. The OR staff may request additional instruction when coordinating the inservice session(s). 6. The Government will award a firm fixed priced contract to the responsible offeror whose offer, conforming to the requirements in this request for proposal, will be most advantageous to the Government, price and other factors considered. A best value award will be made based upon the following factors: Technical, Past Performance, and Price. a. Technical: ability to provide products which conform to the salient characteristics and requirements listed in this solicitation b. Past performance. Identify a minimum of three purchase orders, preferably two from VA Medical Centers, of hospitals currently using offered products, as well as any private/commercial hospitals using offered products which have purchased the products within the last three years. Past performance evaluations will include discussions with medical centers/hospitals regarding the effectiveness of the tools. The Government may obtain and evaluate Past Performance information obtained from sources other than those identified in the quote. c. Price IX.
