THIS IS NOT A REQUEST FOR COMPETITIVE PROPOSALS. The Department of Veterans' Affairs, James A. Haley Veteran's Hospital (JAHVH) located in Tampa, FL intends to solicit and negotiate a sole source requirements contract with BAXTER HEALTHCARE CORPORATION. Statement of Work Baxter Home Choice/Home Choice Pro Automated Peritoneal Dialysis Units Scope and Purpose: This Statement of Work (SOW) is to provide a leased equipment agreement for Baxter Home Choice and/or Home Choice Pro Automated Peritoneal Dialysis (APD) units by Baxter Healthcare Corporation for the James A. Haley Veterans Hospital, Tampa, Florida. Each unit is to be provided with a Home Choice Pro Carrying Case. The lease shall include all installation, maintenance, service, and upgrades as necessary and appropriate. Location: These devices will be provided to inpatients and outpatients of James A. Haley Veterans Hospital to provide peritoneal dialysis as prescribed by a physician for life essential dialysis services. Procedure: Peritoneal dialysis services will be managed by the Nephrology and Renal Care department, and APD cyclers will be provided to inpatients and outpatients to provide peritoneal dialysis services for these patients. For inpatients, there is typically a requirement for one or two of the basic Baxter Home Choice APD units. These devices are required on a short notices basis and must be maintained at the James A. Haley Veterans Hospital. Nephrology staff will manage these units for inpatient use. The requirement for outpatient use varies by patient demand. However, there is typically a requirement for 12-16 units at any given time. Once a physician's order has been given for automated peritoneal dialysis, the patient will use the Baxter Home Choice Pro unit to perform the APD procedure as ordered by the physician. Use of the device will continue indefinitely until there is a change in the patient status or the physician changes the treatment orders. Upon delivery of the physician's order to the vendor, the vendor shall deliver the APD device to the patient's home and provide instructions for use, cleaning, and user maintenance as required. Data Management: The Baxter Home Choice Pro uses a patient compliance card to program the treatment plan and record patient use information. Patients will be provided a compliance card to be inserted into the Baxter Home Choice Pro device provided by the contractor, and the compliance card will be used to program the instrument settings as well as record therapy information. Patients will be instructed to bring the compliance card with them to follow-up appointments for review and therapy adjustments as ordered by the physician. The data on the compliance card will be interfaced with a computer and card reader in the Nephrology and Renal Care department at the James A. Haley Veterans Hospital for reading, interpretation, and programming of the APD cycler. At this time there is no interface through Vista Clinical Procedures for interfacing clinical data to Vista Computerized Patient Record System (CPRS). However, future requirements and interface options may develop that may allow the data to be directly interfaced with Vista and/or Vista Imaging. The proposed contract action is for supplies or services for which the Government intends to solicit and negotiate with only one source under the authority of FAR 6. 302-1. interested persons may identify their interest and capability to respond to the requirement. This notice of intent is not a request for competitive proposals. However, all information received by 10: 00 am EST, 03/05/ 2012, will be considered by the Government. A determination by the Government not to compete with this proposed contract based upon responses to this notice is solely within the discretion of the Government.
