The Department of Veterans Affairs, Iowa City VA Medical Center, Iowa City, IA intends to negotiate a sole source procurement under the authority of FAR 6.302.1, with Volcano Corporation to provide an S5i Imaging system/Volcano IVUS System to the facility. A determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government.This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is VA-263-08-RP-0230 and is issued as a request for proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-25, effective April 22, 2008. The associated NAICS code is 423450 and small business size standard is 100 employees. The acquisition is not set-aside and is being negotiated as a sole source procurement. The requirement for the Volcano s5i Imaging System includes the following: QTY CATALOG NO. A copy of the End User License Agreement is also available from your VOLCANO representative or online at www.volcanocorp.com. 1 Three (3) Year VH IVUS Maintenance Agreement This signed Agreement provides for the purchase of the VH IVUS Software Support and Maintenance Agreement, which provides for unspecified VH IVUS software revisions released during for a three (3) year term (should any be commercially released) at no additional cost.
