This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 -- Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR -- Part 13 Simplified Acquisition Procedures; and supplemented with additional information included in this notice.
This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-87, dated March 07, 2016.
Solicitation VA262-16-Q-1094 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award.
This procurement will be issued under a Service Disabled Veteran Owned Small Business (SDVOSB) Set-Aside. NAICS Code is 339112 and size standard is 1,000 Employees.
The Government is seeking a Contractor that can is seeking sources that can provide a brand name or equal to product; M00546650 SpyGlass DS, a Single Operator & Single Use Cholangioscopy System.
All equal to items must meet the following Salient Characteristics:
1. Must enable therapeutic endoscopist to evaluate indeterminate biliary strictures.
2. Must allow for direct visualization and enhances the ability to easily recognize intraductal pathology.
3. Must be a 10 F miniature endoscope to permit direct visualization of the bile and pancreatic ducts. (A camera, due to its small size, is passed through the working channel of the endoscope.)
4. Must be a catheter-based system for use by a single operator.
5. System must include enhanced digital visualization with 4X the resolution and 60% wider field of view.
6. Must include an automatic light control & LED illumination. The catheter must include a 1.2 mm working channel and two dedicated 0.6 mm irrigation channels.
7. Must be a 10F miniature endoscope that has a channel within it that allows for the passage of a biopsy forcep for tissue acquisition, electrohydraulic lithotripsy (EHL) probes and laser probes.

Provisions 52.212-1, Instructions to Offerors - Commercial and 52.212-2, Evaluation - Commercial Items are applicable to this acquisition. Furthermore, Provision 52.212-2 is utilizing the Lowest Price Technically Acceptable (LPTA) on an Acceptable/Unacceptable evaluation process and award will be made to the offeror who conforms to the solicitation and represents best value to the Government.
Submitting Quote: Offeror(s) shall submit their quote on company letterhead and shall include the completed pricing forms, along with name, address, telephone number of the offeror, firm's DUNS#, total price and a completed copy of FAR Clause 52.212-3 Offerors Representations and Certifications - Commercial Items or a copy of offeror's annual offeror representations and certifications (ORCA) document from www.sam.gov as required by FAR Clause 52.212-3 Offerors Representations and Certifications - Commercial Items. As an alternative, Offeror(s) may use the Price/Cost Schedule of the attached SF 1449 for quote submission.
