This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. It is the intent of the Syracuse VA Medical Center 800 Irving Ave. Syracuse, NY 13210, to negotiate a sole source procurement with Synthes, Inc., 1302 Wrights Lane East, West Chester, PA 19380. This requirement is to provide One (1) Synthes Cervical Retractor Set, including: One (1) Cervical Retractor Frame - longitudinal (U22-640-10), One (1) Blade Ejector (UU22-647-11, One (1) Cervical Distractor, left (2 Pieces)(U44-621-10), One (1) Cervical Distractor, right (2 Pieces)(U44-631-10), Four (4) 14mm Screw (Pin)(U640-14), One (1) Screwdriver (for pins)(U44-642-20), One each Medial Blades (U22-632-40 (23x40), U22-632-45 (23x45), U22-632-50 (23-50), U22-632-55 (23x55), U22-632-60 (23x60)), One each Lateral Blades (U22-634-40 (23x40), U22-634-45 (23x45), U22-634-50 (23x50), U22-634-55 (23x55), U22-634-60 (23x60)), and One each Lateral Blunt Blades (U22-642-45 (23x45), U22-642-50 (23x50), U22-642-55 (23x55). ___________ The Synthes Cervical Retractor Set is provided on a commercial basis, but the only source of this specific brand of equipment is Synthes, Inc., the offeror and incumbent contractor. No other sources can provide the Synthes Cervical Retractor Set. This procurement is being conducted under the authority of Far 6.302-1. Only one responsible source and no other equipment or training will satisfy agency requirements; therefore, full and open competition is determined inappropriate for this acquisition. This notice is for information only and may represent the only official notice of such solicitation/synopsis, however, the Government will consider all proposals received on or before due date identified below. A determination by the Government not to compete this proposed procurement based on the responses to this notice is solely within the discretion of the Government. The provision at 52.212-1, Instructions to Offerors - Commercial Items, applies to this solicitation. The provision at 52.212-3, Offeror Representations and Certification - Commercial Items, applies to this solicitation. The contractor shall return a completed copy of this provision with its proposal. A copy of the provision may be attained from http://www.arnet.gov/far/. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions - Commercial Items. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition. The offeror shall also provide its Commercial and Government entity code, DUNS number, and Tax Identification number. As of October 1, 2003, all Contractors, both large and small, are required to register in the Central Contractor Registration (CCR) database before an award.
