Travis AFB intends to award a sole source purchase order to AMO USA Inc. of Santa Ana, CA for the following: 1) Twelve (12) VISX Custom Vue TX Cards, PN#0030-4835. Each pack included 20 packs of custom Added Vue Cards, and 10 treatment cards. In accordance with Federal Acquisition Regulation (FAR) 6.302-1(a), full and open competition need not be provided when there is Only One Responsible Source and No Other Supply or Service Will Satisfy Agency Requirements. The VISX treatment cards are to be used in the VISX Excimer Laser serial # 5147. The laser refractive surgery is governed by a licensing agreement that excludes all other products from substitution.
