The Department of Veterans Affairs Medical Center, VISN 6 Network Contracting Office (NCO), on behalf of Veterans Administration Medical Center Richmond, is conducting market research through this sources sought notice to seek sources to provide maintenance and support for the Volcano s5i IVUS system at Hunter Holmes McGuire VA Medical Center, 1201 Broad Rock Blvd, VA 23249.
A. General Requirments
Contractor to provide maintenance and support for the Endoworks system the Hunter Holmes McGuire VA Medical Center, 1201 Broad Rock Blvd, VA 23249.
B. Maintenance
Vendor shall provide all necessary equipment and software maintenance and repair as appropriate through the contract period. Maintenance Vendor shall provide a service contract with a response time within one hour for email and phone support between 9 am and 8 pm EST Monday through Friday. This includes unlimited support requests. On-site support is to be within 48 hours and is covered during normal working hours 8am-5 pm Monday through Friday. Onsite service includes all labor, travel and parts /material costs. Replacement parts will be furnished with new parts only including the Patient Interface Module (PIM). Onsite e service requests are unlimited through this contract. Vendor will incur all shipping and freight charges. Preventative Maintenance will be scheduled in advance during the months they are due and performed during normal working hours between 8am and 5 pm on an annual basis not to exceed 12 months in between each PMC.
C. Coverage Period
Maintenance contract to cover the period from 1/01/13 - 09/30/13.
D. Option Years
This contract could include base year (FY 13) plus four option years through FY 17.
