B.2 Statement of Work NCO 10 Nitrile Examination Glove Procurement VHA requests up to or less than $7.5 Million worth of Nitrile Examination Gloves as per specifications listed below. This is not a request for manufacturing but a request for quantity on hand to be delivered CONUS within 30 Days from order, to not exceed $7.5 Million. Quotes submitted with Shipping: Free On Board (FOB) Origin terms, or that require any type of advanced contract financing or advanced payment will render a quote not technically capable and not eligible for award. Similarly, a quote that offers an item(s) that cannot be returned to the vendor or that requires a restocking fee for returned items will be determined as not technically capable and therefore not eligible for award. The Government has the right to refuse acceptance of nonconforming items. Contractors who are awarded contracts based on submitted quotes will have 30 calendar days to deliver the awarded quantities, or the contract will be terminated for cause and negative performance will be reflected within the Contractor Performance Assessment Reporting System (CPARS) and the Federal Awardee Performance and Integrity Information System (FAPIIS). Powder free gloves may only contain a maximum of 2mg per glove. MTR 4 Glove material is nitrile MTR 5 Non-sterile MTR 6 Single use MTR 7 Ambidextrous MTR 8 Puncture Resistant.  Meets ASTM D412 Tensile Test: at least 14 MPa and a minimum of 500% elongation MTR 9 Meets ASTM D6319 standard for nitrile examination gloves for medical application or Meets ASTM D5250 for polyvinyl examination gloves for medical application or Meets ASTM D3578 and ASMT D6977 for synthetic polymer gloves for medical applications MTR 10 Meets ASTM D1671 Blood-Borne Pathogen Test for barrier to the Phi-X174 virus  MTR 11 Meets ASTM D5151 Water Test AQL of 2.5 or lower The attached document titled, PPE Source Questionnaire Nitrile Exam Gloves must be completed and submitted with the offer.
