Cost Per test Agreement: Contractor shall provide (1) iQ 200 ELITE Automated Urinalysis System which consists of (1) ARKRAY AUTION MAX AX-4280 Automated Urine Chemistry Analyzer and (1) IQ 200 ELITE Automated Microscopy Analyzer. 1.1. The contractor shall include all consumables (i.e., reagents, test kits, controls) utilized during the operation of the instruments. Contractor shall provide all maintenance to keep equipment running 7 days per week, three shifts per day. All expenses incurred to keep equipment maintained will be included in cost per test pricing. Service maintenance will be provided within 24 hours of notification of malfunction and training of two operators will be provided. 1.2 Reagent for urine chemistries and urine microscopics shall be included in the cost per test price. These prices include reagents and consumables, which include controls and calibrators for a test volume of 2281 urine chemistries a month and 1217 urine microscopics a month. Additional charges for urine chemistries above 2456 and and urine microscopics above 1521 will be provided. The lab will receive 175 no-charge control and repeat runs for the AX-4280 and 304 no-charge control and repeat runs on the IQ200 per month. 2.0. Replacement of Dysfunctional Equipment: The equipment will be fully functional at all times, all parts, labor and travel are the responsibility of the contractor and will included in the cost per test pricing. 3.0. Service Maintenance: On-site service will be provided during the business hours of 8:00 AM ? 5:00 PM Monday through Friday. A 24 hour technical support line shall be provided for the term of the contract. 4.0. Preventive Maintenance (PM): Laboratory personnel will be responsible for all routine preventive maintenance of equipment, upon training of maintenance by contractor. 5.0. Training: Operator training is included for two key operators: Operator training will include all training manuals, instructional videos, course materials, airfare, lodging and meals.
