Overton Brooks VA Medical Center located in Shreveport LA, requires a Modular Cardiac Cath Lab system and associated equipment (hereinafter referred to as a modular unit) necessary for VA staff to perform invasive cardiac procedure scans. The unit shall include a Philips Allura mobile system, a Mac Lab Hemodynamic system, and a Medrad Advanta Injector, which shall be delivered F.O.B. Destination to the Overton Brooks VA Medical Center, 510 East Stoner Avenue, Shreveport, La. 71101-4295. This agreement shall include, but not be limited to, all labor, material, tools, preventive maintenance and repairs, equipment, delivery, hookup, leveling of the unit and start-up of the equipment and the complete removal of the mobile unit upon conclusion of the contract period. Contractor shall authorize the use of the equipment by qualified VA staff for the purpose intended. Mobile unit will be delivered and removed on a date and time mutually agreed upon between the COTR and the Contractor. Contractor will be provided a minimum seventy-two (72) hours notice when the mobile unit is no longer required. If the return of the unit is anticipated to be in the middle of a normal workweek, the Contractor price will be prorated and price will be decreased accordingly. Contract is for a four (4) month period, to include two (2) one (1) month option periods; total contract period shall not exceed a six (6) month period.
