Schriever Air Force Base has a requirement for a Personal Wellness Profile System which they intend to award sole source to Wellsource, Inc. This system consists of one each of the following: (1) Personal Wellenss Profile Comprehensive Plus software system; (2) Personal Wellness Profile Comprehensive Plus NCS Format Optical Scanning Module, and (3) NCS Opscan 6 Optical Mark Scanner (auto feed/high volume scanner). This system will be used in the Schriever Air Force Base Health and Wellness Center (HAWC) and is intended to provide uniformity and compatibility with the other HAWCs in the area. Any offers received outside Wellsource, Inc., will be rated based on technical and end product aspects. Price may be considered only after technical and end product comparisons have been completed. Any offer of a product which is not on a current General Services Administration (GSA) contract must include a completed Representations and Certifications (FAR 52.213-2).
