This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the solicitation, quotations are being requested and a written solicitation will be issued as request for quotation number H94002-08-Q-0012. Tricare Management Activity intends to award a fixed price order for maintenance services in support of the Xerox copiers described in the Request for Quotation at the address listed below. Request for Quotation #H94002-08-Q-0012 has been assigned to this requirement.
THIS SOLICITATION incorporates the provisions and clauses in effect: FAR clauses 52.203-6, 52.211-6, 52.212-1, 52.212-3, 52.212-4, 52.212-5, 52.213-2, 52.213-3, 52.214-34, 52.214-35, 52.217-3, 52.217-4, 52.217-5, 52.217-8, 52.217-9, 52.219-6, 52.228-5, 52.232-19, 52.232-33 and 52.247-34, DFARS clause: 252.232-7003 and North American Industry Classification System (NAICS) #811212 applies.
PROPOSED PERIOD OF PERFORMANCE: The period of performance shall be for a one year base period from the April 1, 2008 through March 31, 2009, and two one year options from April 1, 2009 through March 31, 2010 and April 1, 2010 through March 31, 20011.
DESCRIPTION OF SERVICES: The Contractor shall provide all maintenance support to include labor and parts, and all consumables, with the exception of paper and staples, for the government owned Xerox copiers under the CLINS and at the location set forth in Request for Quotation H94002-08-Q-0012. (Note: All parts and consumables become the property of the government once they are installed in the copiers.) Maintenance services shall be performed by factory trained technicians, and documentation certifying this training shall be made available to the government. The Contractor shall respond to customer service calls Monday through Friday (with the exception of federal holidays) between the hours of 7:30 a.m. and 4:30 p.m. mountain time. In response to the services calls the Contractor shall provide telephone technical support from a qualified technician within thirty minutes from receipt of the service call. If the malfunction can not be resolved through the support call, a technician shall perform on-site support services within 24 hours from the receipt of the service call unless a portion of the response time falls on a weekend or federal holiday. If this occurs, the response time shall continue into the first business day after the weekend or holiday. And if the copier can not be repaired within 14 business days from the date of the service call, the Contractor shall provide at no additional cost to the government a temporary replacement machine with at least the same capabilities as the government copier until the government owned copier is repaired.
CONTENTS OF QUOTATIONS: The quotations shall consist of a technical quote and cost quote. The technical quotation shall describe how the vendor intends to satisfy the requirements described in the above description of services, and at least three examples of past performance for similar services. The examples shall include a short description of the services, the value of the services, and the name, telephone number and email address of a point-of-contact of the government or commercial representative where the services were provided. The cost quotation shall be submitted on the Government provided REQUEST FOR QUOTATION, Standard Form 18. The fixed price shall be broken down by the Contract Line Item Numbers, (CLINS) for the base period as well as each option period. The technical and cost quotes shall be submitted under a cover letter. The cover letter shall at a minimum include the name, mail address, phone number and email address of the contractor representative who has the authority to commit the contractor to the terms and conditions set forth in the order. The cover letter shall also confirm that the contractor is actively registered on the Central Contractor Registration site at www.ccr.gov.
SUBMISSION OF QUOTATION: Quotations shall be delivered to Brenda L. Agner at TRICARE Management Activity, 16401 East Centretech Parkway, Aurora, Colorado 80011-9066, no later than close of business, 4:30 P.M., March 24, 2008. The quotations may also be delivered electronically via email address
[email protected]. All emails and other correspondence relating to this request for quotation shall have the Solicitation H94002-08-Q-0012 indicated in the subject block.
SOURCE SELECTION: The award will be based on a best value determination, including cost and other factors. Cost will be rated higher than technical and cost and technical combined shall be rated higher than past performance.
AVAILABILITY OF FUNDS AND FUTURE REQUIREMENT: This request shall not be construed as an award, or a promise to award an Order for the requirements. Once a determination has been made that it is in the best interest of the Government to proceed with this procurement and funds are available, an order will be awarded to the selected contractor.
