Acting on behalf of the National Heart, Lung, and Blood Institute (NHLBI), Clinical Trials and Surveys Corporation (C-TASC), the medical coordinating center for the Pediatric Hydroxyurea Phase III Clinical Trial (BABY HUG) under contract no. N01-HB-07160, is issuing the attached Request for Quotation (RFQ). The award resulting from this RFQ will be a 4.5-year contract for a Specimen Core Laboratory to provide a laboratory for analysis of specimens collected in the Follow-Up Study to the BABY HUG Clinical Trial. C-TASC is soliciting quotes (on a fixed price per specimen basis) for a laboratory to perform two blood tests, two urine tests, and prepare blood and transfer specimens for long-term storage at an NHLBI Repository. You are invited to submit a quotation for the attached requirements. Please deliver an original and four copies of both your Technical Proposal and Cost Proposal on or before 4:00 p.m. on February 4, 2008, to:

Please submit separate business and technical proposals. You should address all items in the Statement of Work and include supporting material to demonstrate that your facility is capable of meeting the standards necessary for high-quality biomedical research projects. The award will be made on the basis of price and demonstrated capability to accurately perform these readings. Technical evaluation score and price will have a greater weight than past performance. The evaluation will be based on your demonstrated capabilities in relation to the needs of the project as set forth in the RFQ. Your proposal must document the feasibility of successful implementation of the requirements of the RFQ. The accompanying cost proposal should demonstrate how the fixed prices are determined. C-TASC reserves the right to make awards to offerors whose proposal provides the best overall value to the Government.
In your quotation, please designate and provide contact information for a project director and a business representative to receive regular communications.
If you plan to submit a proposal, please return the attached Intent Form as soon as possible to receive any amendments or other information which may be relative to this RFQ. Doing so does not obligate you to submit a proposal.
