This is a combined synopsis/solicitation for Analyzer and Reagents to test for Chlamydia Trachomatis (CT), Neisseria Gonorrhea and Trichomonas Vaginalis and Human Papilloma Virus (HPV), prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a separate written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) FAC 2005-95, (Effective, 15 June 2018).

The Regional Health Contracting Office - Atlantic, Army Health Contracting Activity, US. Army Medical Command intends to solicit, negotiate, and award a contract for all equipment associated with this requirement that meet or exceed the Government's specifications. The Contractor is to provide the equipment listed in CLINs Structure. All equipment must be compatible with the current existing equipment. Interested Offerors are invited to submit a quotation via e-mail to [email protected] no later than specified above. Offeror shall provide a proposal in accordance with the requirements stated in CLINs Structure. Offerors shall provide ALL items listed. All proposals received after the date of publication of this synopsis will not be considered by the Government.

FAR provision 52.212-1 [Instructions to Offerors Commercial] applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered in the System for Award Management (SAM). The SAM can be accessed via the internet at https://www.sam.gov/portal/public/SAM/. Confirmation of SAM registration will be validated prior to awarding a contract. The provision at FAR 52.212-2, Evaluation - Commercial Items and any addendums to this provision apply to this acquisition.

Vendors shall include a completed copy of FAR provision 52.212-3 Alt 1, Offeror Representations and Certifications with their offer. Offerors shall include a confirmation of receipt of any amendments to this solicitation. Failure to submit a confirmation could result in the offer being determined non- responsive.
