This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

The Navajo Area Indian Health Services, Chinle Comprehensive Health Care Facility (CCHCF), Radiology Department is requesting for submittal of offers for a Non-Personal Service Contract for Anesthesiologists from Date of Award through 10/31/2017, with the option to extend to 12/31/2017. Services shall be performed in accordance with the attached Performance Work Statement (PWS) 09-02. Refer to the PWS for Shift hours. The assignment is through the stated period of performance or until the position is filled with a permanent hire. In such instances, the provider will be released from contract assignment.
