In partial settlement of a case, the Agency seeks to purchase (through a single payment following award of the contract) and own an annuity/insurance policy for the benefit of a former employee ("the primary beneficiary"). The policy must pay the primary beneficiary for the rest of her life the monthly sum of $1,065.00 beginning October 1, 2009, with a 3% COLA added in each year on the anniversary date of the policy's purchase. It must also provide a guaranteed payment of the same monthly benefit to a beneficiary of the primary beneficiary's choosing ("the secondary beneficiary") for a period of 10 years from the date of purchase of the policy, to be paid to the secondary beneficiary only in the event of the primary beneficiary's death within the first 10 years the policy is in effect. For example, if the primary beneficiary were to die in the 7th year of the policy, the secondary beneficiary would receive until the 10th anniversary of the policy's purchase date the amount the primary beneficiary would have been entitled to. The specific parameters are as follows:

Start date of the policy: October 1, 2009

Guarantee Type: Certain Period
Certain Period: 10 (ten) years

Income: $1,065.00 per month for 1 year from the start
date of the policy, then a 3 (three) percent
COLA to be added to the prior year's benefit
each year thereafter

The policy should allow the annuitant to have taxes withheld if she so chooses, or not.
