This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is F3Z4530195A001; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-49 effective 24 Jan 11, DPN 20110302 effective 02 Mar 11, and AFAC 2011-0214 effective 14 Feb 2011. TRAVIS AFB INTENDS TO AWARD A SOLE SOURCE REQUIREMENT TO DAVITA

The requirements are as follows:

CLIN0001- Estimate 8 patients a year with 5 treatments per lot (unit of issue is lot with a lot representing 1 patient). Each patient may have multiple treatments that consist of the units as noted in the list below.

- Plasma Exchange Services (up to 3 hours)
- Cancellation before set up (only if costs incur)
- Cancellation after set up (only if costs incur)
- Waiting Time (per 1/2 hour)
- RN Consultation (per 1/2 hour)
- Plasma Exchange Services (greater than 3 hours) per 1/2 hour increments
- Additional Apheresis Tubing Set during a treatment if the first one must be replaced Apheresis
- Cartridge Change
- TPA Administration (per 1/2 hour)

CLIN 0002 - CAPD and or CCPD per treatment visits, estimate 8 patients a year, with a 7-10 day admission for each patient. This pricing must include all equipment, supplies, labor with a minimum of 2 nursing visits per day.
