This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation N68096-15-T-1013 is issued as a request for quote (RFQ). The North American Industry Classification System Code (NAICS) for this procurement is 339112 (Surgical and Medical Instrument Manufacturing) using a SBA designated Small Business Size Standard of 500 employees. This RFQ is being issued as a small business set-aside under NAICS Code 339112. The US Naval Hospital, Guam is seeking a quote for the items attached. Solicitation and evaluation of offers are to be IAW FAR 52.211-6: Brand Name or Equal. Minimum desired specifications are requested for items which are considered to be equal to or better than the line items listed. Any Award Made as a Result of this Solicitation will be made on an All or Nothing Basis.
