(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Part 13 with FAR Part 12.6 Streamlined, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

(ii) The solicitation number is HE1260-2178-8000 and issued as a request for proposal (RFP).


(iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-55, 2 February 2012. It is the contractors' responsibility to be familiar with applicable clauses and provisions. Clauses and provisions can be found at: http://farsite.hill.af.mil/vffar1.htm

(iv) This requirement is being conducted using full and open competition; there is no set-aside. The associated NAICS code is 423450 and small business size standard is 100 employees.

Audiometer Specifications:
* Meets or exceeds ANSI S3.6 1989, (ASA 81-1989) ISO 389-1985, C.H.D.P, O.S.H.A, E.P.S.D.T., F.A.A., Police, Drivers requirements.
* Frequency range: 250, 500, 1000, 2000, 3000, 4000, 6000, 8000 Hz Accuracy better 3%, (steady and pulse tone).
* Hearing threshold range: 0-90 dB in 5 dB steps Accuracy better than 2dB
* Harmonic Distortion - less than 3%
* Power: 110-220 VAC, 50-60 HZ UL, C.S.A. approved power supply
* Total weight less than 5 lbs. (Complete with headset, earphones, and carry bag.)


(vi) FOB: Destination for delivery to: Multiple delivery locations. Please see Attachment 1 for delivery locations.


(vii) The provision at FAR 52.212-1 Instructions to Offerors Commercial Items, applies to this acquisition. There are no addenda to the clause.

(viii) The provision at FAR 52.212-2, Evaluation-Commercial Items, applies to this acquisition. The Government will award a Purchase Order resulting from this solicitation to the responsible offeror whose offer conforms to the solicitation and whose offer proposes the lowest priced brand name or equal product listed in the attached Exhibit of items.

(ix) Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer; however, if the offeror has provided the required information electronically as part of its annual representations and certifications to the Online Representations and Certifications Application (ORCA), then the Offeror shall instead include a statement in its offer that the provisions at FAR 52.212-3 and DFARS 252.212-7000 for the firm are available online at https://orca.bpn.gov.

(x) The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. There are no addenda to the clause.

(xi) The clause at FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Item applies to this acquisition. In addition, the following provisions and clauses apply under FAR 52.212-5: FAR52.204-10 Reporting Executive compensation and First-Tier Subcontract Awards; FAR 52.222-3 Convict Labor; FAR 52.222-19 Child Labor-Cooperation with Authorities and Remedies; FAR 52.222-26, Equal Opportunity; FAR 52.222-36 Affirmative Action for Workers With Disabilities; FAR 52.225-13 Restrictions on Certain Foreign Purchases; FAR 52.232-33 Payment by Electronic Funds Transfer--Central Contractor Registration; The clause at DFAR 252.212-7001 Contract Terms and Conditions Required to Implement Statues or Executive Orders applicable to Defense Acquisition of Commercial Items applies to this acquisition. In addition the following clauses apply under 252.212-7001: DFAR 252.232-7003 Electronic Submission of Payment Requests; DFAR 252.247-7023 Transportation of Supplies by Sea, Alternate III. The clause at DFARS 252.204-7004 Required Central Contractor Registration.

(xii) The following provisions and clauses apply to this acquisition: FAR 52.233-3 Protest After Award; FAR 52.247-34 - F.o.b. Destination; FAR 52.252-2 Clauses Incorporated by Reference.

(xiii) There is no assigned DPAS rating.

(xiv) All quotes must be emailed to Miyoko Hokama at [email protected] Quotes are required to be received no later than 22:00 Japan Standard Time JST, Thursday, 27 September 2012.

(xv) Request for Quote (RFQ) information and amendments will be posted to the Federal Business Opportunities website at www.fbo.gov. Offerors may email written questions requesting clarification of the RFQ to the Contract Specialist, Miyoko Hokama at: [email protected]. No information concerning this solicitation or requests for clarification will be provided in response to telephone calls.
