THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY. This is not a solicitation for proposals, proposal abstracts, bids, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Responses to the information requested will assist the Government in determining the appropriate acquisition method. The Great Lakes Acquisition Center is planning to solicit for a vendor to provide the following service for the Milwaukee VA Medical Center and Clinics: The Vendor must have the necessary equipment and personnel to provide services as specified under this source sought and shall meet all Federal, State and Local codes and requirements for the operation of equipment. The requirement is for a comprehensive outpatient and inpatient satisfaction measurement service for the Clement J. Zablocki Veterans Hospital (to include the Emergency Department) and Clinics in Milwaukee, Wisconsin. The service will consist of the development of inpatient and outpatient satisfaction survey instruments that include customized questions for the Milwaukee VA facilities as well as questions that permit comparison to other VA and non-VA facilities. The primary goal is to improve patient satisfaction and care by having a service that provides real-time feedback, identifies the root cause of dissatisfaction, and offers solutions for progress. The contractor will provide all services associated with preparation, distribution and collection of the survey instrument and data. The contractor will be expected to use quantitative and qualitative data analyses in accomplishing the tasks and providing quarterly written reports, which will include action plans and recommendations to improve patient satisfaction. The Great Lakes Acquisition Center/ VA Milwaukee is conducting a market survey to determine the existence of potential small business concerns (e.g., HUBZone, 8(a), SDVOSB, Woman Owned, etc.) with the capability to accomplish this Service Interested parties shall provide a brief overview of their firm s capabilities and past experience with services similar in scope to this service. All information shall be submitted in Word or PDF format, not to exceed 10 pages, including all attachments. Each submittal shall also include the following business information: DUNS number Organization Name Organization Address Point of Contact (including name, title, addresses, telephone number, fax number and Email address) Business size Socio-economic status (IF SDVOSB or VOSB provide SDVOSB or VOSB CVE verification) Firms shall identify whether or not they are a small business or other than small business in accordance with the size standard for NAICS code 541611.
