We wish to enter into a 48 month agreement for the purchase of BioCare supplies for use with "IntelliPATHfix" made by BioCare. The costs include a monthly commitment of $4,375 for supplies, freight for shipping the equipment, equipment maintenance in years 2-4 at $8,500 per year with an increase of up to 5% in years 3 and 4. Additionally, to compensate for increased business, we are requesting to spend an additional amount of supplies in the amount of $42,000 per year which we are already spending now with only one machine.
