A) The OCB consists of three (3) buildings. The buildings are comprised of Building (A) Resident Dormitory-2 floors which houses one (1) elevator; Building (B) Administration Building, and Classrooms; Building (C) Cafeteria/Kitchen. The facility¿s residential dormitory is available for up to 36 participants that reside on-site for the duration of their program. Adult participants attend on-site classes and training from 8:00 a.m. to 4:00 p.m. Monday through Friday. B) Type of Service 1) The Contractor shall provide Elevator Service, Maintenance & Repair, for the Department of Rehabilitation, Orientation Center for the Blind (OCB) at the address listed below in accordance with the scope of work. 2) Timely services must be provided to ensure limited interruption of business operations. C) Location/Service Area Department of Rehabilitation (DOR) Orientation Center for the Blind (OCB) 400 Adams Street Albany, CA 94706 D) Service Hours All Services shall be provided seven (7) days a week, excluding holidays, between the hours of 7:00 a.m. to 5:00 p.m. Contractor shall contact the Contractor Administrator upon arrival and completion of work. The OCB Business Hours are from 8:00 a.m. ¿ 5:00 p.m. Monday through Friday. The majority of service calls will be during normal business hours from 8:00 a.m. ¿ 5:00 p.m. Monday ¿ Friday. However, after hours and weekend calls could occur on occasion. E) Estimates The Department of Rehabilitation¿s Contract Administrator and/or designee has the authority to request the Contractor to provide an estimate prior to authorizing any services. F) Response Time The Contractor shall respond to any call within a minimum of four (4) hours from the time in which a non-emergency callback was reported to the Contractor. For emergency calls or callbacks such as ¿entrapments¿, the Contractor shall respond within a minimum of one (1) hour from the time the emergency was reported to the Contractor. For elevator shutdowns involving passenger entrapments, a comprehensive report shall be prepared and submitted to the OCB within 24 hours of the entrapment. The report shall include elevator identification, date and time of entrapment, time entrapped passengers were released, cause of entrapment and corrective action taken by the Contractor. G) Call Back Services Should the Contractor fail to provide any of the specified services in the prescribed manner the State may call the Contractor to provide services within four (4) hours, unless otherwise specified at no additional cost to the State. All work shall be under warranty for one (1) year from the date of completed repair. Time and costs for callbacks to previous repairs under warranty shall be borne by the Contractor. H) Notification The Contractor will notify the Contract Administrator of required service and/or scheduled times and dates for regular maintenance. I) Verification of Arrival The Contractor/service technician is required to have the location staff verify time on the job site by reporting to the front desk and signing in at the time of arrival/signing out at the time of departure. Location staff must be on site while the service is being performed. Location staff has the authority to determine whether the service performed requires the services of more than one (1) service technician. The Contractor shall leave a copy of the work order signed by the location staff indicating: 1) time of arrival; 2) time of departure; and, 3) general description of work performed at the location. J) Parts and Materials Contractor shall replace, without charge, parts with a dollar value of twenty-five dollars ($25) or less which have become broken or worn through normal use and are necessary to keep the equipment fully operational. For cost reimbursement of parts over $25, the Contractor must submit a common parts price list or the Contractor will be required to submit original receipts. For cost reimbursement of parts/materials $100 or more, original receipt(s) will be required. The charges for parts and materials shall not exceed twenty percent (20%) of the Contractor¿s wholesale costs; excluding tax, (i.e., $100 parts cost x 20% x 9.75 (City Sales Tax) = $131.70). Only original manufacturer parts and materials shall be used. Removed parts shall be left at the location for State inspection. REFURBISHED OR REBUILT PARTS ARE NOT ALLOWED K) Freight Charges Contractor will be allowed to recover freight or shipping costs for parts, with no allowance for mark-up by: 1. Itemizing the cost of freight or shipping on the invoice; 2. Attaching a copy of the freight or shipping invoice (original whenever possible) to the invoice; and, 3. When the freight or shipping invoice includes costs not specifically associated with the billing, the Contractor will only charge for the costs associated with the billing and note the freight or shipping invoice accordingly. L) Work Area Contractor will ensure that the work area is kept clean and free of debris, as necessary, to maintain a safe working environment for staff. While working on equipment Contractor agrees to perform services with as little disruption to the State¿s operations as possible. All tools, equipment and other work materials belonging to the contractor will be removed from the work location at the end of each working day. The State shall not be responsible for storage of any contractor property. M) State Property 1. It shall be the Contractor¿s responsibility to make all necessary arrangements with office/building manager and local traffic authorities for use of elevators, loading docks, adjacent streets, sidewalks, and/or malls. 2. Any damages by the Contractor to portions of buildings, premises, equipment, furniture, material or other property for which the State is responsible will be repaired or items will be replaced by Contractor to the satisfaction of the State with no expense to the State. Contractor shall repair, replace and/or finish in kind all surfaces of features displaced, or damaged in performance of work, such as but not limited to acoustical tile, floor covering, and wall areas. 3. Contractor shall provide dust covers, temporary dust barriers, pads, etc., as required to protect building occupants and contents during its operation. Upon completion of the work, there shall be no discrepancy between the new and existing work. Where existing surfaces are painted, under the provisions of this paragraph, paint shall extend to the nearest natural break. 4. Contractor shall paint or finish all new work, except factory finished items, a minimum two (2) coats to match existing adjacent area. Any items lost or stolen while in the Contractor¿s custody will be replaced by the Contractor at no expense to the State. Contractor must receive prior approval from the State to remove equipment from a location. 5. Floors, walls, and ceilings of buildings, including elevators, will be adequately protected by Contractor to prevent marring and scratching or other damages. The State may, itself, at its option, repair any damage or replace any lost or stolen items and deduct the cost thereof from Contractor¿s invoice. 6. Contract shall keep site clean as work progresses. Upon completion of the work, all surfaces involved in this project shall be clean of any foreign material. Contractor shall remove all waste materials from the work site and dispose of same daily. N) Commencement and Termination of Contract The service to be performed under this contract shall begin on the date specified and continue for a period of five (5) years. The State may terminate this contract at any time by giving the Contractor at least thirty (30) days written notice of its intention to do so. O) Insurance See Exhibit D for Basic Insurance Requirements. Proof of current insurance certificate with limits stated in Exhibit D for Commercial Liability, Automobile Liability, and Workers¿ Compensation, which must be submitted prior to a contract award. P) Notification Contractor shall not interrupt any building services without securing prior approval from the Contract Administrator. ¿ Contractor agrees to provide a five-year (5) Full Service Maintenance and Repair per Thyssen Krupp Elevator, Maintenance and Owner¿s Manual to the one (1) Thyssen Krupp Elevator, Model #TAC32, located at 400 Adams Street, Albany, CA 94706, for the Department of Rehabilitation, Orientation Center for the Blind. Additionally, incorporated in this Elevator Service, Maintenance & Repair contract; the contractor agrees to provide inspection and corrective maintenance that may be needed as directed by the Department of Industrial Relations (DIR) certification requirement(s) and/or a DIR preliminary order(s). Contractor agrees to be licensed, insured and has permits according to local, state and federal code requirements. Contractor agrees to provide a license/certified technician to provide service and maintenance of the one car, permissible load 2500 lb. hydraulic elevator. Contractor shall furnish all supplies, rags, materials, cleaning materials, solvents, oil, grease, lubricants, tools, equipment and labor, and every other item of expense necessary to perform Elevator Maintenance, Inspection and Repair services specified herein for one Thyssen Krupp Elevator. Contractor agrees elevator shall be operational and tested upon completion of each service. Contractor agrees upon completion of service/maintenance that written documentation of services performed shall be provided to the contractor liaison or her/his designee. Contractor agrees to and will follow the periodic inspections and tests of conveyances as recommended by American Society of Maintenance Engineers (ASME) A17.1-2007, Section 8.11.1.3. Frequency of maintenance intervals of conveyances shall be as recommended by ASME A17.1-2007, Section 8.6 referenced and incorporated as part of the contract agreement. Elevator Fire Recall Test ANSI/ASME A17.1, Rule 1206.7 and shall include but not be limited to the following: IN THE CAR ¿ Observe acceleration, deceleration and leveling ¿ Observe operation of doors and gates ¿ Check call button, position indicators and lanterns; door protection device operation, condition of components ¿ Test alarm bell, telephone and emergency light operations ¿ Replace all lamps ¿ Verify anti-evacuation devices ¿ Lubricate guide rails ¿ Inspect car top IN THE PIT ¿ Check pit stop switch, ladder and lights ¿ Inspect bottom of car and cylinder packing ¿ Check oil return lines and pit bucket ¿ Clean pit floor and equipment IN THE MACHINE ROOM ¿ Check pump, motor, tank and controller ¿ Empty drip pans ¿ Clean machine room equipment and floor REMOTE ELEVATOR/EMERGENCY TELEPHONE MONITORING Contractor agrees to provide twenty-four hour per day, seven days per week remote monitoring. Emergency Telephone monitoring shall be all inclusive per this agreement. PARTS Replacement parts shall be as outlined in the manufacturer¿s Maintenance and Owner¿s Manual recommendations. Contractor agrees no aftermarket or reconditioned, or refurbished parts shall be used. GUARANTEE Regardless of kind or quantity, all materials and workmanship to be furnished and performed under this contract shall be fully guaranteed against defects for a period of one (1) year unless otherwise specified, after date of completion and acceptance of the contract. RECORDS/LOGS/DOCUMENTATION REQUIREMENTS All inspections, lubrication, adjustments, tests, cleaning, routine repairing, and other maintenance activities shall be performed with a written receipt of the work completed by the Contractor and include any noted deficiencies and suggested corrective action. Contractor shall maintain an accurate and complete log of all work performed in addition to routine inspections. These logs shall include emergency callback services describing the nature of all complaints and their resolution. The logs shall be kept in the Equipment Room at the site. ACCIDENTS/DAMAGES The Contractor shall be liable for damages arising out of injury to a person and/or damage to the property of the State, employee of the State, and any other person(s) other than agents or employees of the Contractor, designated by the State for many purposes, prior to, during or subsequent to delivery, installation, acceptance and use of the deliverables either at the Contractor¿s site or at the State¿s place of business, provided that the injury or damage was caused by the fault or negligence of the Contractor.
