GENERAL DESCRIPTION:

A Firm Fixed-Price, Non-Personal Health Care Service contract for Cardiology Services to be performed at the Pine Ridge I H S Hospital, E. Hwy 18, Box1210, Pine Ridge, SD 57770. Period of Performance will be from Date of Award thru October 31, 2024. The contractor is to perform according to the attached Statement of Work. The quoted "unit price" is an all-inclusive cost. This is a Base plus Four (4) Option years.

The "Unit Price" is an all inclusive cost. All "inclusive cost" is defined to include travel, lodging, per diem, fringe benefits, i.e., life insurance, social security, federal, state, and local taxes, plus all other costs pertinent to the performance of this purchase order/contract/DO/TO.

BASIS FOR AWARD: Quoters are advised that the Government will use price in determining the best value for the government to determine the successful contractor. Quoter's are advised to provide a Certificate of Liability Insurance with their quote and the required government forms need to be completed and submitted with the bid packet to be considered a viable bid.

5. Technical Evaluations
6. Contractor Forms, OF306, Waiver, licensure, Child Care (If these forms are not received in the bid packet then the packet is incomplete). These need to be completed by all staff that will be performing services on this award.

OIG Exclusion List - No contract award shall be made to any vendor or provider listed on OIG Exclusion List - http://exclusions.oig.hhs.gov throughout the duration of the contract. Should the candidate be found with a non favorable security clearance it shall be the contractor's responsibility to replace the candidate working under this contract with a suitable candidate. It shall be the responsibility of the contractor to notify the Acquisition Office if there is a change in provider.

The selected offeror must make arrangements with the Service Unit Background Investigation Coordinator and complete the following as soon as possible:

- Fingerprints
- SF-85 Questionnaire for Non-Sensitive Positions (done electronically)
