Provide a price and lead time quotation with signed Representations & Certifications by not later than May 25, 2016.

Source selection will be based on lowest price method and the product delivery date must be within the month of August but not before August 1.

Freight terms: FOB Destination

The resulting purchase order will be issued on a fixed price basis. SLAC National Accelerator Laboratory Terms & Conditions for Commercial Supplies and Services under $100,000 M365 (Rev 17Feb2015) will be made a part of the resulting order.
