Notice of Proposed Contract Action:
This is a notice of intent to award a sole source contract. The award is to be a Firm-Fixed Price contract pursuant to Federal Acquisition Regulation (FAR) 13.106-1(b)(1)(i), "Contracting officers may solicit from one source if the contracting officer determines that the circumstances of the contract action deem only one source reasonably available." The contracting officer has determined that only one source is reasonably available: Airborne Medical Depot, LLC, a SDVOSB, located at 5464 W Lady Bug Ct, Tuscon, AZ 85742, the only known source capable of meeting the requirement.

The Department of the Air Force has a requirement for Centinel Spine's Midline II Ti spinal implants and associated supplies as follows:

- Qty 2: M331108C Midline II Ti 33mm x 11 x 8 degree
- Qty 8: STM6030 STALIF Midline ABO Screw
- Qty 2: STM6030R STALIF Midline ABO Screw Revision
- Surgical sales representative shall hand deliver items prior to scheduled procedure.
- Surgical sales representative shall provide guidance during procedure and have stock on-hand to meet patient needs as determined by surgeon during the procedure.
