THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION AND A SEPARATE SOLICITATION WILL NOT BE ISSUED. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-71 (November 2013). This announcement constitutes the only solicitation. The solicitation number for this acquisition is HHSNCCOPC14-005644sol and is being issued as a Request for Quote (RFQ). The NAICS Code for this solicitation is 423450 and the Small Business Size Standard is 100. This will be awarded as fixed price. The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular FAC 2005-71 effective September 30, 2013. The associated North American Industrial Classification System (NAICS) code for this procurement is 423450 and the Small Business Size standard is 100 employees. This combined/synopsis solicitation is not a set aside for small business. The acquisition will be made pursuant to the authority in FAR 13 to use simplified procedures for commercial items.

REQUIREMENT SPECIFICATIONS


The Department of Radiology and Imaging Sciences requires the procurement of 17-gauge ablation probes for use with Neuwave Certus 140TM 2.45 GHZ ablation system.

Provide the Certus 140TM 2.45GHz Ablation System Accessories, including the patented LK and PR probes, which are customized for Liver and kidney and predicatble ablation zones.

1) The probes interface with a single 2.45 6Hz signal source generator and has three (3) independent power amplifiers, each capable of producing up to 140W each.
