Locate and pump/remove 3 cesspools at Camp Hale Koa TMK (4) 1-4-02:55 Waimea Canyon State Park. Locate and pump/ remove 2 cesspools at the United Church of Christ TMK (4) 1-4-04:27 Kokee State Park. Locate and pump/remove 1 cesspool at Kamokila village TMK (4) 2-03-5 Wailua River State Park. Contractor is responsible for 1. filling and completing the application for backfilling an injection well cesspool along with applicable fees. 2. Using a geologist or qualified professional engineer to determine the appropriate method for backfilling an injection well cesspool. Once the determination is made, the geologist or professional engineer is responsible for the monitoring, assuring, and endorsing that the backfilling work was appropriately completed. 3. A completion report signed by the geologist or professional engineer must be submitted to the Department of Health's Underground Injection Control program for file closure. Use the Injection-Well Cesspool Backfilling Final Completion Report form that will be mailed to you.
