University Health is seeking proposals for administration of: 1. Flexible Spending Accounts (FSA) for Health and Dependent Care 2. Consolidated Omnibus Budget Reconciliation Act (COBRA) continuation for medical, dental, vision and FSA accounts 3. Retiree billing for medical, dental and vision premiums 4. Affordable Care Act (ACA) 1095 reporting and form distributions Each service will be reviewed separately. Vendors may submit a proposal for one or more of the services or for all four. The administration of the Flexible Spending Accounts should include the following: • Debit cards • COBRA continuation for FSA accounts • Carrier or Broker must attend all Open Enrollment Fairs • Regular employees must be eligible to participate in the FSA accounts on the 1st of the month following 30 days of employment • House Staff employees are eligible to participate in the FSA accounts on their date of hire or the 1st of the month following their date of hire The administration of COBRA should include the following: • Must allow grandfather status and direct transfer of all existing participants • Must assume responsibility for distribution of initial COBRA notices to new hires and changes (i.e. qualifying events) • Must offer administrative services for medical, dental, vision and the FSA accounts The administration of Retiree Premium Billing should include the following: • Must collect Retiree premiums for medical, dental and vision The administration of ACA 1095 reporting should include the following: • Must generate the 1095 forms for all participants to include COBRA participants, Retiree participants, former employees and active employees • Must distribute the 1095 forms to all participants • Must file the information with the IRS • Must provide a consolidated report of what was submitted to the IRS
