TRICARE Management Activity is a Government organization and is exempt from Federal taxes. If the service is available via your GSA Schedule, identify the GSA Schedule Number, ordering terms, conditions, period of performance, include the pricing sheet from your GSA Schedule. Pricing can be lower than the GSA price, but it cannot exceed the GSA amount. If quoting "open market", identify the items that are "open market", complete the attached Representations and Certifications and include it with your response.

Submit your pricing on the format provided. DO NOT modify the format or submit your priced quote on an alternate format. In submitting your pricing the quantity, times the Unit Price equals the Amount.

SUBMISSION OF QUOTATION: Quotations shall be delivered to Brenda L. Agner at TRICARE Management Activity, 16401 East Centretech Parkway, Aurora, Colorado 80011-9066, no later than 1:00 P.M. Mountain Time, November 17, 2008. The envelope containing your quotation must be clearly identified with the quotation number H94002-09-Q-0002 and the requirement name of Convention and Lodging Services. Electronic or faxed quotations will not be considered for award. In addition to your priced quote you will need to also include responses to all information that has been requested in the Request for Quotation package.

Quotations received after the date, time and method stated above may be rejected. All correspondence relating to this request for quotation shall clearly display Solicitation Number H94002-09-Q-0002.
