Furnish all labor, materials, services, equipment, and facilities necessary to fabricate and deliver the following:

QUANTITIES OF 14 EACH CsI Crystals per attached specification "PRODUCTION CRYSTAL SPECIFICATION FOR Mu2e" dated August 1, 2016 V3.0.

Unilateral Option No. 1: 675 each of CsI Crystals to be exercised no later than 1 year after receipt of initial order quantity.

Unilateral Option No. 2: 1,450 each of CsI Crystals to be exercised no later than 1 year after receipt of initial order quantity.

The award of this "best value" subcontract will be based upon a Technical Evaluation and a Price Evaluation. The evaluation criteria, in descending order of importance, is as follows:

Technical Evaluation, consisting of:
- Technical Merit
- Capabilities, Infrastructure and Experience
- Schedule
- Manufacturing and Quality Assurance

PLEASE SUPPLY ENOUGH INFORMATON ON THE ABOVE CATEGORIES FOR FERMILAB TO CONDUCT A THOROUGH EVALUATION.

Please keep Technical Proposal separate from Price Proposal.

It is our intention to issues multiple order for the initial quantity of 14. After receipt and performance review of initial deliveries, an award will be made for the production quantities as outlined in the options above. Based on performance, there will be one to two vendors selected for the production quantities.

Offeror shall quote unit prices PLUS TOTALS.

Offeror to quote all in US DOLLARS only.

Offeror to package components adequately to prevent damage while in transit to Fermilab.
