The purpose of this amendment is to extend the response date to 9:00pm CST on August 6, 2015, and to provide the answers to the various questions received. 1. Item 0001, is this a single POTS line with local service only? (i.e. FACP, alarm) No we have long distance. We additionally have an ADT alarm connected to one of the lines. We don't have any wireless connections. 2. Item 0002, with regard to the 200 DID numbers, is there a need to port any specific numbers? Possibly, because since those circuits belong to AT&T we may have to port the numbers over from them if another vendor comes in and replaces with their own circuits. 3. Item 0002, is it a correct assumption that all DID numbers will use the ISDN PRI for transport? Yes. 4. Item 0003, what is the address of the FE Building at the VA Medical Center (the other end of the point-to-point circuit)? Lakemont Data Center floor ground room 134 Physical address: 2500 S Lakemont Ave, Orlando, FL 32814 Sim Learn NSC floor ground room 018 Physical address: 6490 Hazeltine National Drive Suite 120, Orlando, FL 32822 77.LOXX.777392.UFLG (CTL 5. Item 0004, Will this PRI be terminated into an existing PBX? Yes, The PRI connects to a blade in the PBX managed by the VAMCORL IT. It is 23b+1d and has 23 lines. The NEC PBX yields to glare and we are currently receiving 4 digits. VAMCORL IT Telecom is not sure if you we are getting your own PBX or using service from the Lake Nona Hospital PBX. The expansion was a chassis that houses 6 blades the blades added are for phones, we could add additional blades for PRI's, phones etc. 6. Is there an ETA on when the services need to be installed by? For this question, it will depend. If a company aside from CenturyLink wins the deal, would they have to install all of their own equipment? Or is it just establishing services? Whichever is the case, how long should that take (to make the transition between providers)? It will have to be expedited as quickly as possible. There will have to be notification and the Management informed of how long the system will be down to port all the numbers.
