Dental Hygienist

Adams and Associates, Inc. contracting for the U.S. Department of Labor for the operation of the Treasure Island Job Corps Center - a 532 student residential training facility located at 351 Avenue H, San Francisco CA 94130, is currently soliciting written bids for a Dental Hygienist.

All quotes must be submitted in writing to the Purchasing Department. Quotes received after the deadline and/or without all requested documentation will not be considered.

SCOPE OF SERVICES

Dental Hygienist services will be provided in the amount of 16 hours per week on Mondays and Tuesdays. 

•·         Management of periodontal diseases

•·         Oral hygiene instruction

•·         Diet and Wellness counseling

•·         Placement of dental sealants

•·         Scaling and root planning

•·         Performing periodontal evaluations

•·         Charting

•·         Taking x-rays

The Base Period of Performance will be November 1, 2015 and the end date July 31, 2016 and August 01, 2016 to July 31, 2017.

In addition to the above period of performance, additional pricing shall be provided for 3 option period of performance listed: August 1, 2017 through July 31, 2018, August 1, 2018 through July 2019 and August 1, 2019 through July 31, 2020.

Bids will be due by Monday August 31, 2015 at 1pm
