Guthrie Job Corps Center, operated by ResCare, Inc. under contract J10F6OK016 with the Department of Labor/Employment and Training Administration, is seeking to Subcontract for the services of a Dental Hygienist.
The Center Dentist Hygienist is an independent contractor, and not an employee, representative, or agent of the Guthrie Job Corps Center. The Center Dental Hygienist will provide services not to exceed thirty-two (32) hours per week, during a schedule determined by the Center Dental Hygienist and the Manager of the Wellness Center to meet the health requirements of the center enrollees.
QUALIFICATIONS: Certifications, licensing or accreditation as required by the State of Oklahoma. Experience in providing dental care for adolescents and young adults. Provision of dental services to youths who are economically and socially disadvantaged. Willingness and ability to carry out duties and responsibilities equal to those specified in the attached sub-contract.
Period of Performance for this agreement is July 1, 2011 through June 30, 2012. Contract value: $50,000 - $75,000.
