Bldg 5801 will be permanently closed and no longer operational at close of business, 4:00 pm on 9 Nov 2011. The new hospital facility, Bldg 1094, will be open and fully operational beginning 0800 on 15 Nov 2011.

If you are awarded this contract you will be required to move all of your Contractor Furnished Equipment to include all consoles and any other equipment necessary for the performance of this contract to the new facility. You are responsible for moving all of your equipment after closure of Bldg 5801 after COB on 9 Nov 2011 and prior to the opening of the new facility on 15 Nov 2011.

The government will not be responsible for relocating or any additional expenses incurred during the transition to the new facility.

The Occupational Medicine Facility located in Bldg 3334 will not be affected in this move.
